Contact and Referrals
Meet the team
We have two palliative care Consultants, a Clinical
Psychologist, three Nurses, and an Administrative Assistant. We
will usually meet children and their families on the hospital wards
or outpatient department. We have specialist knowledge and will
work supportively with your child's healthcare teams to find
ways of providing care that best reflects your family's values
and preferences.
Referring to
our team
We accept referrals from hospital based and community health
care professionals. We also accept self-referrals from patients and
their families but please note we will also need to contact your
treating team. We will endeavour to respond to referrers within two
working days.

Working together
We are always keen to hear from families that have used our
service and want to share their ideas and experiences about how we
can continually develop and improve going forward. With this in
mind we are seeking to identify families who would be willing for
us to contact them in the future to invite you to consult on
service developments/projects and/or attend consultation events as
they arise.
If you feel that you may be able to help us with this work
and/or would like to receive further information we would be
grateful if you could complete the consent
form and provide us with your up to date contact details. We
will then be in contact with you about individual projects and
events as they arise to invite you to be involved.
