Introduction to Transition

What is it?

Now you are growing up there will be lots of changes in your life: you will have some decisions to make about what you want to do at school, what job you would like in the future and where you want to live. There will also be changes with your healthcare. Your team will start talking to you when you are about 13 years old about taking more responsibility for managing your condition and making decisions. When you are between 16 and 18 your care will be transferred to the adult hospital or for some conditions, your GP. The preparation for and the transfer from the children's hospital is called transition.

We want to help you develop the confidence and skills to manage your healthcare. This website will give you information about your health condition, where you can go to get information and support about health and lifestyle issues and news about events and resources that might interest you.  It will hopefully provide answers to some of the questions that you might have and about adult services across the region.
