Transition

General Info

To visit our general transition homepage click here.

This site has been designed to offer support and information to young people who have been diagnosed with a medical condition and are recieving treatment at University Hospitals Bristol and Weston. If you were diagnosed as a child or young person you may be trying to understand your condition more, manage its impact on other areas of your life and have, or be about to transfer to an adult hospital. As there are lots of different medical conditions we thought we would create these pages to help you start on this process. You can find more specific info on speciality pages. If there isn't one for your condition yet then let us know if you want to help us make one! This website has been created with young people who have experienced this journey and is based on areas they feel would be helpful to other young people.

You can use it to find out how other people felt, to explore your own thoughts and feelings and to access information and support.

You'll find:

  • Information
  • Useful links
  • Things to try
  • What support there is in your local area

As a young person you will be learning to manage you condition more independantly and think about how it impacts on other areas of your life. There is lots of information here to help and support you with this. You might find some of it helpful to talk through with your healthcare team or parents and carers.

If you have any questions, concerns or comments about your experience as a patient or have questions about your condition or the hospitals and don't know who to contact you can phone our LIAISE team on 0117 3428065 or email BCHinfo@UHBW.nhs.uk.

The website is a work in progress and will develop based on your feedback, so if there is an area missing, or you have found something helpful that isn't covered please do let us know on the contact details above.
