photograph.
You may have a rash or a mark on your skin that we
have asked you to photograph.

If you have a mark:
Please take the images according to the attached guidance (/media/1839224/telederm_photos.pdf)

put an ink arrow pointing to the mark on your skin, or mark a
ring around it
·
ask someone to help take the photographs so that they can get
the mix of distance and close up views
o
ensure that there is good, even light in the room. If
not, use the flash
o
take a view of the region of your body: eg leg, back, arm,
face such that the inked mark is clear within the image
o
take a close up view of the area: no closer that 10cm/4
inches, press on the screen at the site corresponding to your mark
so that the yellow square forms and locks focus on the
area.
o
repeat this view at an angle to show if the mark is
raised
o
take a photo of the appointment letter you have received or
alternatively of your own name and date of birth which you have
written on a piece of paper

If you have a rash

you may not need to highlight the rash if it is obvious, but
if not, please use a marker on your skin to indicate the
rash

ask someone to help take the photographs so that they can get
the mix of distance and close up views
o
ensure that there is good, even light in the room. If
not, use the flash

take a view of the region of your body: eg leg, back, arm,
face such that the inked mark is clear within the image
o If
the rash is symmetrical, eg both elbows or both shins, take a view
that captures both of these together to show this.
o
take a close up view of the worst area: no closer that 10cm/4
inches, press on the screen at the site corresponding to your mark
so that the yellow square forms and locks focus on the
area.
o
repeat this view at an angle to show if the rash is
raised
o
take a photo of the appointment letter you have received or
alternatively of your own name and date of birth which you have
written on a piece of paper

review the images and make sure they are in focus

send the entire package of photos, choosing the maximum quality
whilst remaining within 10MB for the entire image set.

send the images to the email address
