Who are we?

Community Children's Nursing Team

The CCN Team are a team of experienced Paediatric Nurses with specialist skills and experience in nursing children and young people at home, as well as in other community settings.

What do we do?

Aiming to prevent admission to hospital, promote early discharge and prevent readmission.

Examples of Care Include:

Intravenous Antibiotics
Wound Care
Injections
Oxygen Monitoring
Central Venous Access Device Care
Monitoring / Review of Medical Conditions
Support with delivery of/ compliance with treatment regimes

This is a 24 Hour answerphone, monitored frequently during the day. Messages left after 5:30pm (Mon-Fri) or 3.30 pm (Sat/Sun/Bank Holidays) will be dealt with at 8am the following day. If leaving a message please give your name and contact telephone number with a short message.

Nursing care can be provided to children registered to a GP within the Bristol and South Gloucestershire CCG areas.
