Transition in the Cystic Fibrosis service
Transition is a process which aims to prepare you for managing and living with Cystic Fibrosis independently, whilst providing any support and information you need.
You will find information on these pages for you both if you have already moved to adult services and if you are just starting transition and haven't yet transferred.
If you have just started transition you might be wondering what will happen. As a young person you will find the Cystic Fibrosis team will start to talk and discuss your health and treatments with you directly and for some of your appointments we would encourage you to be in the room without your parent/carer. This doesn't mean they shouldn't be included in supporting you but just gives you and the Cystic Fibrosis team an opportunity to talk about and start managing your health independently.
Transition will cover aspects such as:
- Your knowledge about Cystic Fibrosis
- Your treatments and medications
- Cystic Fibrosis complications
- Taking increased responsibility and control of your health
- Living with Cystic Fibrosis
- Lifestyle choices - including diet, alcohol, smoking and sexual health
- Information about the adult service

We understand, for some, that transition with regards to your health may feel like a daunting idea while there are lots of other decisions to be made linked with becoming more independent.
