To refer a child to the Children's Hearing Centre team please download and fill out this form. 

Make sure you save the completed form then email it to paediatricaudiologyreferrals@uhbw.nhs.uk. 

Auditory processing disorder guidance for referrers 

Please see the attached document, with guidance for those referring children and young people who present with difficulties listening, and may be suggested to have Auditory Processing Disorder (APD).
