Patient demographic and contact data
Name of physician referring (unless
private)
Diagnosis
MRI (Imported into NBT PACS)
Other recent investigations as appropriate
e.g. endocrine function if pituitary tumour; hearing tests if
acoustic neuroma; staging scan of primary and other metastases and
performance status if intracranial metastases.
Date and outcome from prior MDT
Expected prognosis, if intracranial
metastases, from extra cranial disease
Other treatments previously given for
condition
Additional information ideally
available within referral:
Co-morbidity
Medication and allergies
Performance status
History of condition including symptoms
Relevant social information
Preferred contact method for patient

Names of other consultants involved in the

All referrals will be discussed at MDT and
then if suitable will be referred to the Cinical
Oncologist.
