What is Supportive and Palliative Care?
Supportive and Palliative Care Team helps patients and families
with serious illnesses at any stage of their disease particularly
if symptoms are having a negative impact on quality of life. We aim
to help patients live as actively as possible by providing high
quality pain and symptom control as well as psychological, social
and spiritual support.
Specialists in Supportive and Palliative Care Team have
expertise in supporting patients and their families who are living
with a life limiting conditions. They work alongside your medical
team to enable high quality individualised care at the point of
need.
We aim to:
- Focus on a team approach to physical, psychological, spiritual
and social concerns by working alongside other staff who remain
responsible for patient care.
- Provide opportunities for patients and families to discuss
their wishes and help plan future care.
- Provide continuity of care by improving liaison with outside
professionals and community Palliative Care Services.
- Develop education, audit and research in Palliative Care across
the hospital trust to ensure high quality standard of Palliative
care is being provided.
How to access The Supportive and Palliative Care Team?
- In most cases your medical team will make a referral to
the hospital Supportive and palliative care team for management of
pain and difficult symptoms. Nursing staff can also refer provided
this has been agreed by the medical team.
How quickly will I be seen by Supportive and Palliative Care
Team?
- We aim to see patients in hospital as
quickly as possible - we will try to see the same day or by the
next working day.
