Community care
In the community end of life care is led by the GP working
together with the community nursing service. There may also be
other teams involved in caring for people nearing the end of their
lives such as the services below.
Bristol Care Coordination Centre for End of
Life
The organisation coordinates care for patients in the last three
months of life across Bristol and provides the following
services:
- Palliative Care Home Support Service:
To provide personal and nursing care to patients in the
Bristol and South Gloucestershire area who wish to be supported in
their last days/weeks of life, and to enable them to stay and
die at home. The team provide nursing care for patients and support
to family members. The team can visit daily and are able to
increase the number of visits as required. The team also work
alongside other healthcare professionals like community nurses, GPs
and hospice nurses. Referrals can be made by any health or social
care professional;
- Fast Track Service: Assess patients who
are rapidly deteriorating on a daily basis for funding to
cover a package of care for them to remain at home or a nursing
home placement in Bristol.
- Marie Curie service: Allocates Marie
Curie nurses to provide night care for patients in the last three
months of life.

Hospices are charitable organisations that care for local people
who have incurable illnesses. The focus of care is to improve
patients' quality of life, while extending care and
support to their families and loved ones. They offer many services
