University Hospitals Bristol NHS Foundation Trust and Weston Area
Health NHS Trust.

How to request information from us under the Freedom of
Information Act
- All requests for information must be made in writing, clearly
stating what information is being requested, and including the
requestor's name and address/email address, in order for us to
respond.

- Your request will be acknowledged and you can expect to receive
the requested records (if held) within 20 working days.
- The Trust may ask you for further information, which it
reasonably requires in order to identify and locate the information
that is requested.
- In some cases, information may be exempt from disclosure. If
some of the information is of personal and confidential nature, it
will be excluded as will any other confidential material.
- Material relating to the health and safety of specific
individuals, as to law enforcement or criminal or regulatory
enforcement or audit issues under certain circumstances may also be
excluded from publication.
- There may be circumstances where material cannot be released
because the appropriate officer of the authority has taken the view
that it may be prejudicial to the conduct of public affairs.
- Should this be the case, then we will state the reason why the
information cannot be released and the course of action to appeal
if not satisfied.
Cost of information
University Hospitals Bristol NHS
Foundation Trust will not charge for fulfilling standard
information requests from individuals. However, if the Trust
estimates that a request would cost more than £450 (staff time
is calculated at £25 per hour regardless of grade/actual rates) to
complete, then as a pubic authority it is are not required to meet
such a request.
In these cases, the Trust can offer the
requestor advice on ways in which they could adjust their request
to bring it under the cost limit.

Complaints related to Freedom of Information
Requests
If, after receiving a response to a Freedom of Information
Request, you are dissatisfied with the handling of your request,
you have the right to ask for an internal review. Internal review
requests should be submitted within two months of the date of
receipt of the response to your original letter and should be
addressed to: Trust Secretary, University Hospitals Bristol NHS
Foundation Trust, Trust Headquarters, Marlborough Street, Bristol,
BS1 3NU.
