**May 2024** Referrals to the service can now be made electronically by completing a referral form and emailing to: PrimaryCareDentalReferrals@uhbw.nhs.uk

WE DO NOT ACCEPT SELF-REFERRALS TO PCDS. IF YOU THINK YOU SHOULD BE SEEN IN PCDS PLEASE ASK A HEALTH OR SOCIAL CARE PROFESSIONAL (e.g. your GP, health visitor, social worker) TO MAKE THE REFERRAL USING ONE OF THESE FORMS.
