Clinical Outreach Team
The outreach nurses are part of the clinical site
team (CST). Together with the hospital's senior management team,
the team have an overview of the hospital and provide support to
all ward areas.
They are a team of experienced children's nurses
with a critical or acute clinical care background - this means they
have all worked in areas such as intensive care, high dependency or
the emergency department.
Their role is to support and empower the nurses in
ward areas to care for children who are more acutely unwell, or
whose condition has deteriorated and may need extra care.
The team works in partnership with all grades of the
multi-disciplinary team to ensure all acutely ill children in the
hospital have timely and appropriate care, across all of the
hospital wards, ensuring the best outcome for each child.
There are a number of different reasons why the
outreach team may become involved in your child's care:
- If your child's observations, such as heart and respiratory
rate and blood pressure, are outside of normal limits,
- If your child has been discharged from the paediatric intensive
care unit (PICU) to a ward bed, they will be followed up for 48
hours, regardless of whether they are still acutely unwell or
not.
- If your child has been discharged from PICU to high dependency,
they will follow up children discharged from PICU to a high
dependency bed for a minimum of 24 hours.
The outreach nurses will help to ease your
transition to the ward and provide support to you and your child in
settling into this new area.
You may also see them in them in children's
emergency department assisting during emergency situations.
