As part of becoming a young person you might have started to understand more about your condition and as part of becoming more independent it is important to learn how to manage your health. As you grow older you take much more responsibility for this. It doesn't mean that your parents/carers can't be involved in supporting you, just that you have more control over decisions and managing your own health. Your medical team might have mentioned the word transition to you already; this describes the process of supporting you in learning about and taking more responsibility alongside thinking about how it impacts other parts of your life.

These pages have lots of information both for if you have just started transition and haven't yet moved or if you have already transferred to adult services.

 The transition process will include things like:

  • Your knowledge about your metabolic condition
  • Your treatments and medications
  • Taking increased responsibility and control of your health
  • Living with an metabolic condition
  • Information about and support moving to the adult service
