Access/ Referrals

Nerve Conduction Studies can be requested via the ICE referral system under the Neurophysiology link, (this is the preferred method for referrals).  If, however, you do not have access to ICE, a Nerve Conduction Referral form can be requested from the Neurophysiology Department for use within your Surgery / Health Centre. Alternatively referrals can be made by letter and posted, faxed, or  emailed to the Neurophysiology Department. All referrals should include (as a minimum) the name, date of birth  and contact details for the patient, the pertinent clinical details and a return address for the test results.
